Affected customers can claim up to $2,500 for provable out-of-pocket losses resulting from the breach, such as fraud or identity theft, requiring documentation like bank statements or police reports.

Those unable to prove financial losses can claim up to $30, though this amount may be reduced based on the number of claims filed. The deadline to submit a valid claim is December 26, 2025. This settlement follows a similar case involving Integris Health, which agreed to pay up to $25,000 per patient in a separate data breach settlement.
