Individuals who had claims for residential treatment denied between April 29, 2017, and April 30, 2025, may be eligible to claim funds. Eligible customers can receive a pro rata share of the Out-of-Pocket Reimbursement Fund or a nominal payment of at least $100. To claim, individuals must provide proof of out-of-pocket payments made within 14 days of a qualifying denial and submit a claim form by January 20, 2026.
