Anthem agrees to $12.8 million settlement — here’s who can claim a payment

Claimants must have paid out-of-pocket for treatment initiated within 14 days of the denial and submit documentation to support their claim. Eligible class members will receive either a pro rata share of the reimbursement fund or a minimum payment of $100, depending on the number of valid claims received. The deadline for submitting claim forms is January 20, 2026, and the final approval hearing is scheduled for January 26, 2026. By remaining in the settlement, class members waive their right to independently sue Anthem over the same claims.
